To play any of the activities at Sudden Impact you must fill out an insurance waiver form for the activity you wish to take part in. Any player under the age of 18 must have this document countersigned by a parent or guardian. Please ensure that ALL INFORMATION is completed and legible.

RELEASE AND RENTAL AGREEMENT
I AM over 18 years of age and want to play Paintball and sign this document in consideration of being given the opportunity to engage in the activity. All information supplied is to the best of my knowledge correct and without any false representation as at the date this document is signed.

The minimum age to play paintball at Sudden Impact Leisure Ltd  is 12 years of age.

I UNDERSTAND THAT:

1. The paintball event I have booked in for may be physically demanding if I am pregnant, have a weak heart or suffer from any medical condition and I should consult a medical doctor before taking part.

2. The game of Paintball can be dangerous if not played in accordance with the rules, which I have read and fully understand.  If I have not read the rules I will ask to see them before playing.

3. The possibility of injury to others and myself exists.

I CONFIRM AND AGREE THAT:

1. I am fully aware of the risks to myself and others involved in playing paintball and that I will NEVER under any circumstances remove my face mask, except in the designated SAFE AREA.

2. I am physically fit and mentally able to take the strain and exertion involved in playing paintball.

3. I will comply with all Sudden Impact Leisure Ltd’s rules and regulations and will use the equipment as instructed and not so as to injure or hurt others.  I will obey all directions and instructions from the event staff.

4. If I am judged to be acting in a manner which may pose danger to others, or myself, I understand that I may be expelled from the venue and also understand that I will not be entitled to any refund.

RELEASE

I hereby release, remise and forever discharge from any claims and liabilities whatsoever, without limitations, that I may have against Sudden Impact Leisure Ltd, the owner of the property, and any other persons, howsoever caused, that may arise from having attended this event.  I make this release on behalf of myself, my heirs, executors and administrators.

RENTAL AGREEMENT

I agree to pay £250.00 to Sudden Impact Leisure Ltd in the event that the equipment that I hire to play Paintball is not returned at the end of the day to Sudden Impact Leisure Ltd.

I have read and fully understand all the terms of this document and confirm that I am not relying on any statements or representations by any person or entity as inducement to my fully and voluntarily engaging the Paintball Games assuming the risks and obligations stated above and signing this document.
